• Passport size photo (in JPEG)
• payslip 3 months
• Latest 3months EPF
• Efiling Statement from LHDN & e-BE form latest
• Birth cert 
• Doctor's confirmation letter (for sick/disabled parents)

Wife details: -

• Ic Copy
• Passport size photo (in JPEG)
• payslip 3 months
• Latest 3months EPF
• Efiling Statement from LHDN & e-BE form latest
• Birth cert 

Maid details: -

• Nationality of Maid
• Reason of hiring
• Passport copy
• Current visa copy (if currently in Malaysia)
• Thumbprint
• FOMEMA
• Security Clearence (if in source country)
• Passport size photo (in JPEG)

Important Terms: 

• Maid must be Female
• Not less than 21 years old and not more than 45 years old
• Muslim employers are allowed to employ only Muslim Maids
• Maid must do house work only, cannot engage in carwash or cleaning office etc.
• Non-Muslim employers who employ Muslim Maid must allow:
• Doing worship like praying 5 times a day
• Ramadan fasting and
• Not being told to do housework that is against Islam.
• Ensure Maid does not get married in this country while holding Maid Visa
• Maid salary must be paid no later than the last week of each month
• Keep records of Maid salary payments and show them to the Department when requested
• Employers are not allowed to hit or do any act that causes injury to Maid
• The employer is responsible for reporting to the Malaysian Immigration Department if the Maid:
• Die
• Lost
• Run away from work.
