Dokumen Wajib

• Sijil Pendaftaran Syarikat / Perniagaan - Form 9, 24 & 49 atau Form B & D (Terkini dan disahkan dalam tahun semasa/dicetak SSM
• Jika SSM hanya ada kategori 'Stesen Minyak', kategori 'Jualan Barangan Borong/Runcit' perlu ditambah.
• Pendaftaran SSM mesti mengandungi "Jualan Barangan Borong/Runcit"

• Kad Pengenalan Pemilik / Pengarah 

• Syarat-Syarat Bagi Wakil Syarikat:
  - Terdiri dari peringkat pengurusan syarikat
  - Nama tersenarai di dalam Penyata Caruman KWSP terkini
  - Kad Pengenalan pemohon
  - Surat Perwakilan dari majikan (Ditandatangani oleh Pihak Pengurusan Syarikat

• Penyata Caruman KWSP bagi 3 bulan terkini

• Laporan Kewangan Syarikat yang telah diaudit bagi tahun semasa atau setahun sebelumnya

• Penyata Bank bagi syarikat untuk 3 bulan terkini (Berturutan)

• Insurans Skim Pampasan Pekerja Asing (SPPA)(Foreign Workers Compensation Scheme Insurance (FWCS) ) bagi pekerja asing sedia ada (Jika Berkenaan)

• Memo Periksa Keluar bagi pekerja yang telah pulang atau/dan Laporan Pekerja Lari daripada Jabatan Imigresen Malaysia (Jika Berkenaan)

• Lesen Perniagaan yang dikeluarkan oleh Pihak Berkuasa Tempatan

• Rental Agreement or Sales & Purchase Agreement

• Gambar hadapan premis (termasuk papan tanda nama syarikat) & gambar dalam premis

Mandatory Documents

• Company / Business Registration Certificate - Form 9, 24 & 49 or Form B & D (Updated and verified in the current year/printed by SSM
• If SSM only has a 'Gas Station' category, the 'Wholesale/Retail Sales' category needs to be added.
• SSM Registration must contain "Wholesale/Retail Sales"

• Director Identification Card 

• Conditions for Company Representatives:
• Consists of the company's management level
• Name listed in the latest EPF Statement
• Applicant's Identification Card (IC)
• Letter of Authorization from employer (Signed by Company Management)

• EPF Contribution Statement for the last 3 months

• Audited Company Financial Report for the current year or the previous year

• Bank Statement for the company for the last 3 months (Consecutive)

• Foreign Workers Compensation Scheme Insurance (SPPA) (Foreign Workers Compensation Scheme Insurance (FWCS)) for existing foreign workers (If Applicable)

• Check out memo for workers who have returned or/and Runaway Worker Report from the Malaysian Immigration Department (If Applicable)

• Business License issued by the Local Authority 

• Rental Agreement or Sales & Purchase Agreement

• Front picture of the premises (including the company name sign) & pictures inside the premises
